OUR SUPPORT STAFF
Each provider is assigned to a nurse and care coordinator assistant team. They are available Monday-Friday 8:00 am – 5:00 pm. Our staff is very sensitive to the needs of the frail elderly and their families and/or caregivers. They are responsible for the following:
New patient enrollment
Scheduling provider visits
Prior authorization
Specialist referrals
Chronic care management
Care coordination and transitional care
Nursing triage
Medication refills
