WELCOME TO GSC

Geriatric Specialty Care (GSC) is a unique practice with an emphasis on the frail, elderly population providing visits in the patient’s place of residence: skilled nursing facilities, group homes, long term care, assisted living facilities and private residences.

GSC provides services in the following geographic areas:  Reno/Sparks, Carson City/Gardnerville, Fernley, Fallon and Elko.  We have teams of nursing and care coordinator assistants assigned to specific providers who are responsible for care coordination and care management services.

Our clinical and support staff are very caring and compassionate regarding the needs of our population we serve. GSC embraces patient centered care with respect to cultural and religious preferences.

GSC CLINICIANS

We have nine physicians that are board certified in internal medicine, family practice, geriatrics, and/or psychiatry/neurology. One physician is certified in Acupuncture. In addition, GSC has two nurse practitioners who have extensive experience working with the geriatric population and are wound and ostomy certified. All providers are committed to delivering excellent care. You will be assigned one of the providers listed below as your primary care provider.

NURSING & SUPPORT STAFF

The GSC nursing staff is very sensitive to the needs of the frail elderly and their families and /or caregivers. They are responsible for the following:

  • New patient enrollment
  • Nursing triage
  • Scheduling provider visits
  • Prior authorization
  • Specialist referrals
  • Chronic care management
  • Care coordination and transitional care
